Gunya Meta strives to provide support and opportunities to families within Logan Central. To ensure that families and community members are able to access health services Gunya Meta has appointed a Community Connector, which works to close the health accessibility gap.
The Community Connector supports families and community to gain access to services and facilities which are needed to obtain better health.
The Community Connector can:
- provide referrals to services.
- guide families and notify them regarding supports and services available.
- provided transport if needed.
The aim of having a Community Connector is to remove any and all barriers that families and community might face when accessing health services.
